Put all the necessary information specifically the name, date of birth, address – make up an address, current medication list, medical conditions, allergies, vital signs if necessary and include a first line chemotherapeutic regimen from the NCCN guidelines. Once that is complete then provide a patient counseling points as well as education pamphlet of what the patient needs to do in regards to the regimen once she is at home including follow-up information with the pharmacist and/or physician.
Provide a plan of care for her next steps using the NCCN guidelines.
